ELIGIBILITY | All
undergraduate students are required to have a health history and admitting physical
examination on file to use the Student Health Service. These forms are required for
initial admission to each of The Claremont Colleges and are important because they provide
essential information about ongoing health care and past treatment. Failure to supply this
information makes a student ineligible to use the service. Entrance physical exams can be
done at the Student Health Service, for a fee, by appointment. |

RECORDS |
All students’ records are CONFIDENTIAL! Medical records are not made available to anyone (including parents) without the student’s permission. To receive medical records the Student Health Service needs to have a signed and dated authorization telling what to release and where to send the information. Please include the school attended and year of graduation. Include a phone number where you can be contacted in case of questions. Please be advised that a $5.00 copy and mailing fee is required prior to release of information for all Alumni. Payment should be either money order or cash (we do not recommend sending cash in the mail). We do not accept personal checks. For current students a nominal copy fee will be assessed. Contact Student Health at (909) 621-8222 for charges. You
can send your authorization by mail, come in personally or fax your
request to (909) 621-8472. Mailing address is: Student Health Service,
Medical Records 757 College Way. Claremont, Ca 91711.
Note: We do not fax medical records due to federal laws &

INSURANCE | It is strongly recommended that all students be covered by medical insurance to protect against the major costs of a severe illness or accident. Harvey Mudd, Claremont McKenna and Pitzer Colleges require each student to submit proof of coverage prior to registration. Most students are covered under their parents insurance policy. If you are not, you can purchase an insurance policy through the Claremont Colleges. An explanatory brochure with application form is available to all students through the Student Deans Office of your college or at the Student Health Service.
