WORKERS' COMPENSATION INSURANCE
All California employers must provide Workers' Compensation benefits, at no cost to their employees, through insurance or through a State approved self-insurance program. Zurich Insurance Company provides coverage for the Claremont Colleges.
Anyone experiencing an illness or injury resulting from employment may be entitled to Workers' Compensation benefits. Such an injury or illness may have resulted from a single incident or from repeated and prolonged exposure to activities or substances at work. Your benefits may include all medical treatment necessary to cure or relieve you from the effects of a work-caused injury or illness.
WHAT TO DO WHEN INJURED AT WORK:
Report all injuries or work related exposures/accidents to your supervisor immediately (even if you do not wish to receive medical treatment). Your supervisor is required to submit the necessary reports: Supervisor and Employee Report of Accident. You can also contact your Human Resource Office or the Disability & Unemployment Office. Your claim cannot be processed until your employer knows about the injury. Immediate reporting is vital to insure proper medical treatment.
Medical treatment is provided at First Care Industrial Center. You must first obtain authorization from Belinda G. Ochoa, Patty Munoz or your Human Resource Officer before receiving medical treatment for any work-related injury.
Any injury occurring after hours, weekends or holidays notify Campus Security extension 7200.
Unless you have previously pre-designated your personal physician (in writing), all medical care for the first 30 days of treatment must be obtained from First Care Industrial Center. Pre-designating forms are available in the Disability & Unemployment Office or through your Human Resource Office.
If you cannot work because of your work-caused injury/illness, you may be eligible to receive Temporary Disability benefits. An authorized physician must substantiate your disability. The State Law excludes disability payments for the first 3 days off unless you are off work more than 14 days or become hospitalized. The Colleges allow employees to use accumulated sick leave and/or vacation to supplement Workers' Compensation benefits, thus continuing income equivalent to full pay.
If you have any questions concerning your claim for benefits, please feel free to contact Disability & Unemployment at 621-8847 or 607-3943. You may also contact an Information and Assistance Officer who will be able to help you and answer your questions. These offices are located throughout the state in the Industrial Accidents Office, Office of Benefits Assistance and Enforcement.
IN CASE OF SERIOUS LIFE-THREATENING INJURY OR ILLNESS: Call Campus Security @ X 72000
Indicate nature of emergency · Give exact location of victim
Security will call paramedics IF warranted and/or come to the scene and provide the necessary emergency treatment.
The State of California requires that you be provided with the following information:
Any person who makes or causes to be made any knowingly false or fraudulent material statement or material representation for the purpose of obtaining or denying Workers' Compensation benefits or payments is guilty of a felony.
