The forms below must be completed and returned to Rob Fossum before a Club Sport conducts any practices or competitions.

Application (PDF)
To be recognized as a Club Sport, this form needs to be filled out completely and requires signatures from the sport's student leaders, advisor, and coach.

Medical Insurance (PDF)
Each member of a Club Sports team needs to fill out both pages of the medical card.

Consent to participaton and release (PDF)
All members of a Club Sports team must sign this form.
